Benchmark Independent Medical Examinations Inc. (“Benchmark” and “we”) is committed to a Person Centered Philosophy. We have established various policies and procedures to protect your rights. Your Rights:

Confidentiality and Privacy: You have the right to confidentiality and privacy.

Access and Correction: You have the right to request access to, and correction of your personal information.

Dignity and Respect: You have the right to be treated with dignity and respect.

Freedom from Abuse, Exploitation, Retaliation, Humiliation, Neglect or any other Behaviours of this sort: These behaviours will not be tolerated.

Pertinent Information: You have the right to be given pertinent information with enough time to allow you to make decisions.

Consent, Withdraw Consent, or Termination of an Examination: You have the right to withdraw your consent or terminate your participation in an examination at any time.

File a Complaint: You have the right to file a complaint if you feel that our level of service or your experience with us is less than satisfactory, and may fall short of what might be reasonably expected.

Investigation and Resolution: You have the right to an investigation if you feel that your rights have been infringed upon.
